SER are currently working alongside a Mental Health Service based in Greenwich who are currently seeking to appoint a Flexible Community Support Worker to join their team on a permanent contract.
This role involves supporting clients within the community to support them on their individual recovery path, offering flexible assistance for individuals with enduring mental health needs on a 7-day shift rota. The focus is on guiding clients through their recovery journey over a 24-month period, helping them set and achieve their goals, become more self-reliant, and reduce their dependence on medical interventions and hospitalisations.
The role also includes providing outcome-focused support, addressing dual diagnosis issues, managing housing-related aspects, and collaborating across various services and sites as needed.

Contract:
Permanent Contract 37.5hrs per week across a 7-day rota, worked flexibly to suit the needs of the service.
Salary: £23,365.24

About the role:
* Support Clients being discharged from hospital back into their own homes
* To provide flexible support to clients with enduring mental health needs over a shift rota of 7 days a week which will include evenings.
* To support client move on and manage their progress along their individual recovery path within a 24-month period
* To encourage clients to identify and define their own goals and work towards being as resilient and independent of services as possible, reducing their reliance on medical interventions and hospitalisations
* To provide focused outcome-based support measured against clients’ goals, recovery and achievements
* To support clients with specific needs arising from dual diagnosis
* To provide housing management across a range of accommodation provided to clients
* To meet the needs of the service you will be expected to work across different services and sites
Essential
Requirements:
* Experience of working with adults with enduring mental health needs or a similar field
* Experience of supporting client move on to independence within a time defined recovery path
* Understanding of Dual Diagnosis and complex needs
* Knowledge of The Mental Health Act
* Knowledge of Housing Acts and legislation
* Able to work flexibly across 7 days a week on a shift rota basis
